1. Acceptance of Terms and Conditions
By accessing and using the services provided by Home Physio and Training, you agree to be bound by these terms and conditions. If you do not agree to these terms, please either contact us for any clarification, or refrain from using our services.
2. Services Offered
Home Physio and Training (referred to as “Service Provider”) offers physiotherapy, massage therapy, and personal training services (collectively referred to as "Services"). These services are provided by fully registered, qualified, and insured professionals in their respective fields. All services are subject to availability. While we aim to offer appointments as soon as possible and at times that suit you, this cannot be guaranteed
3. Service User Responsibilities
Clients and/or patients (collectively referred to as "Service Users") that use our services are responsible for the following:
o Providing accurate and complete medical and health information relevant to the services they are seeking.
o Following the instructions and recommendations of the Service Provider
o Respecting our policies regarding appointment scheduling, cancellations, and fees.
o Reading the service information provided and giving consent before initiation of services.
4. Payments and Refunds
o For single bookings outside of a package deal, the Service User has the option to pay a £50 booking fee which is the minimum required to secure the booking. The remaining fee for the appointment (overall fee minus the booking fee) will need to be paid immediately after the appointment has finished. There is also the option to pay for the service in full at the time of booking.
o The booking is not confirmed until at least the booking fee is paid.
o For purchasing packages, the whole fee must be paid before the first session. Package discounts do not apply when booking through insurance; they are only available for self funding clients.
o We accept payment by bank transfer only.
o We are recognised by most major health insurance providers. If you are using private health insurance to cover your sessions, please let us know in advance and ensure you have authorisation from your insurer before the session. You are responsible for any outstanding balance not covered by your insurer.
o Late payment may result in additional charges, suspension of services and/or legal action.
o A full refund will be issued if the Service User cancels the appointment with at least 24 hours' notice before the start time or if the Service Provider cancels the appointment, provided the Service User has complied with these terms and conditions.
5. Cancellation Policy
Cancellations can be costly for us, so we operate a strict cancellation policy.
o A full refund will be issued if the Service User cancels the appointment with at least 24 hours' notice before the start time or if the Service Provider cancels the appointment, provided the Service User has complied with these terms and conditions.
o Cancellations made within 24 hours of the appointment start time are considered "Late Cancellations."
o For single sessions, a Late Cancellation will result in the £50.00 booking fee being retained as a “Late Cancellation Fee.” For sessions within a package, there’s no Late Cancellation Fee, but the session will be counted as used.
6. Scan Referrals
We offer access to scans through our partnership with Vista Health. An initial physiotherapy assessment is required before a referral can be made. A referral admin fee of £15.00 applies.
7. Physiotherapy Reports
We can provide written physiotherapy reports to support applications such as Personal Independence Payment (PIP), Blue Badge, prolonged sickness leave, or workplace adaptations. An initial assessment is required, and a £50.00 fee applies. Please note: in most cases, a GP report is preferred over a physiotherapy report. Physiotherapy reports should be used as supplementary evidence only and do not carry statutory or legal authority.
8. Privacy and Confidentiality
We adhere to strict privacy and confidentiality policies regarding the Service User’s personal and health information. Please refer to our privacy policy for more details.
9. Health and Medical Information
Clients are responsible for disclosing all relevant medical information and health conditions to the Service Provider to ensure safe and effective treatment.
10. Liability and Waiver
The Service Provider is not liable for any injuries, accidents, or adverse reactions that may occur during or as a result of our services. Service Users assume all risks associated with our services.
11. Termination of Services
The Service Provider reserves the right to terminate services to any client for any valid reason, including but not limited to non-compliance with these terms and conditions.
12. Changes to Terms and Conditions
We may update these terms and conditions from time to time. The most up-to-date terms and conditions will always be available on our website.
13. Governing Law and Jurisdiction
These terms and conditions are governed by and construed in accordance with the laws of the United Kingdom: England and Wales. Any disputes shall be subject to the exclusive jurisdiction of the courts in the United Kingdom: England and Wales.
